• Name of Applicant and address.

• Telephone and fax numbers.

• Mailing address and phone number of the representative if different than the above.

• Nature of the interest - nature of legal or economic relation to the undertaking or affected properties, or concern with the undertaking's effects on historic properties. [Federal Highway Administration (FHWA) must know the interest to determine if person or organization meets the criteria for consulting party status.]
